INSTRUCTIONS to fill out the membership contract
IMPORTANT: The printed number on the application form, including your initials will be your
Distributor Number (write it down and memorize it).
If you join Omnilife under my sponsorship, please SEND ME A MESSAGE to
keep in touch, and to send you additional information.
- When you get the kit, fill out the membership contract (white page) , SIGN IT, and tear out the yellow page,
this will nbe your copy of the contract.
- You will need to send the original contract (white page) by mail along with a picture ID to the Omilife
distribution center that sent you the kit.
- The membership contract form is divided in three sections; the first section is for your information; the
second section is for a co-distributor info (optional).
- The third section is for your sponsor info, (just write my name and distributor number, my signature is not
necessary.)
- If you need to place an order before you send the contract by mail, you can do so, by faxing a copy of your
contract, and a copy of your ID to the fax number printed in your INVOICE.
- Wait 1 hour, and call 1-888-326 1188 to place your order, and confirm that they have received your
documents.
- If you choose to send a copy of your contract by fax, you still have to send the original copy to Omnilife’s
corporate office (see address above) - NOTE: You do not have to fill out the W8 or W9 forms: they are for
taxes purposes only
